Pauper’s Burial Permit Application

No person shall be buried in a cemetery in the municipality of Montgomery, Alabama until a burial permit is obtained from the City Sexton's Office.

Chapter 7-17 (c) of the City of Montgomery, Alabama Code of Ordinances
"Notice of burial shall be submitted with a burial permit form and an administrative fee in the amount of $125.00 payable to the city to cover the costs of maintaining accurate record of all deaths and burials which occur in the city."

Burial Permit forms for indigent persons that the Montgomery County Department of Human Resources approve for a “Pauper’s Burial” need to be completed, but there are NO charges for the Burial Permit.
Burial Permit forms for infants need to be completed, but there are NO charges for the Burial Permit for children less than one year old.

I understand and agree that in the event this should not be the proper lot or grave space, or if it should be legally determined that said remains should not have been interred in this location or that I did not have the right to authorize this interment in this location, I agree and obligate myself to have said remains disinterred and removed at my expense. I further agree to abide by all the current rules and regulations of the City of Montgomery. I also understand and agree to hold the municipality of the City of Montgomery, Phillip Taunton (City Sexton), and the employees of the City of Montgomery free and harmless in any action or liability whatsoever which may result from their compliance with my request in making this interment on the burial space indicated above.
